The Lonely Brain Ages Faster

A Multimodal Brain Age Gap Study in 725 Aging Adults

1. Overview

Scientific rationale and importance. Loneliness and social isolation shorten life by an estimated 15 years—a mortality impact the U.S. Surgeon General has equated to smoking 15 cigarettes per day^1,2,3. Across 148 studies and 300,000 participants, social disconnection increases premature mortality by 26–32%¹, and independently raises dementia risk by 26% in the UK Biobank (N = 462,619)⁴. Yet the biological mechanism by which loneliness accelerates brain aging remains unknown. We propose that chronic loneliness makes the brain age faster—and that this accelerated aging is driven by a vascular-inflammatory cascade measurable with multimodal MRI. Specifically, we will: (1) compute a multimodal brain age gap (predicted brain age minus chronological age) to directly test whether lonely individuals have biologically older-looking brains; and (2) identify the vascular, inflammatory, and microstructural mechanisms driving this acceleration using three independent MRI biomarkers—arterial spin labeling cerebral blood flow (ASL-CBF), free water fraction (FW), and T1w/T2w ratio—in the openly available HCP-Aging cohort (~725 adults, ages 36–100+).

Funding, facilities, and ethics. This project is entirely computational and requires no new data acquisition, wet-lab resources, or participant recruitment. All analyses will use the publicly available HCP-Aging dataset accessed through the NIMH Data Archive (NDA). T1w and T2w structural data have already been downloaded and processed through the FreeSurfer pipeline by the PI. Computing will be performed on the GPU cluster at the Gruss Magnetic Resonance Research Center (MRRC), Albert Einstein College of Medicine. The PI has existing NDA data access credentials, established diffusion MRI and ASL processing pipelines, and published expertise in both advanced diffusion modeling⁷ and the neuroscience of loneliness⁸. No additional IRB approval is required for this secondary analysis of existing de-identified data (original study approved by Washington University IRB).

Timeline. 6 months. Month 1: Complete HCP-A diffusion and ASL data download; deploy processing pipelines (T1/T2 already processed). Months 2–3: ASL-CBF quantification, free water estimation, and T1w/T2w ratio computation; brain age model training. Month 4: Statistical analyses linking loneliness to brain age gap and regional biomarkers. Month 5: Manuscript preparation. Month 6: Preprint submission, code release, and preregistration update.

Data sharing commitment. All analysis code, processing pipelines, brain age models, and statistical scripts will be released publicly on GitHub under an open-source license. Derived biomarker maps and brain age predictions will be shared through the NDA. A preprint will be uploaded to ResearchHub and bioRxiv.

2. Introduction

The scale of the loneliness crisis is extraordinary. Half of U.S. adults report experiencing loneliness³. Among adults aged 65 and older, one in four is socially isolated⁹. Young adults ages 15–24 reported a 70% decline in time spent with friends over the past two decades³. The COVID-19 pandemic further amplified these trends, with 56% of adults aged 50–80 reporting feelings of isolation during 2020¹⁰.

The health consequences are severe. Loneliness increases the risk of cardiovascular disease by 29%, stroke by 32%, and premature death by 26%^1,11. In one landmark prospective study of over 30,000 men, those who were unmarried with small social networks had a 90% increased risk of cardiovascular death and were more than twice as likely to die from stroke, accident, or suicide¹². Critically, older adults who are both isolated and lonely face the highest mortality risk¹³, suggesting these constructs operate through partially independent but converging pathways.

The loneliness–mortality link extends directly to brain aging and dementia. In the Framingham Heart Study, loneliness was associated with a 3-fold increased risk of dementia over 10 years, lower total cerebral volume, and greater white matter hyperintensity burden—a hallmark of cerebrovascular injury¹⁴. In the UK Biobank, socially isolated individuals showed lower gray matter volumes in temporal, frontal, and hippocampal regions, and these structural differences mediated the relationship between isolation and cognitive decline⁴. These findings suggest that loneliness does not merely correlate with poor brain health—it may actively accelerate the biological aging of the brain.

The Allostatic Stress Model: From Loneliness to Accelerated Brain Aging

According to the allostatic model of social attachment⁵, loneliness functions as a homeostatic signal indicating that one’s baseline requirement for social connection has not been met. When loneliness becomes chronic, persistent activation of stress-responsive systems generates cumulative biological “wear and tear” through multiple converging pathways:

(1) HPA axis dysregulation. Chronic loneliness elevates cortisol and disrupts its diurnal rhythm¹⁵. Sustained glucocorticoid exposure damages hippocampal neurons and endothelial cells, reducing cerebral perfusion.

(2) Sympathetic hyperactivation. Loneliness increases sympathetic nervous system tone, causing chronic vasoconstriction and elevated blood pressure^6,16. This directly reduces cerebral blood flow and accelerates small vessel disease.

(3) Systemic inflammation. Loneliness elevates C-reactive protein, interleukin-6, and pro-inflammatory cytokines^15,17. Neuroinflammation increases extracellular free water in brain tissue through edema, gliosis, and tissue degradation.

(4) Sleep disruption. Loneliness is strongly associated with insomnia and fragmented sleep¹⁸, impairing glymphatic waste clearance and compounding cerebrovascular injury.

(5) Behavioral mediators. Social disconnection reduces physical activity, promotes unhealthy eating, and increases substance use¹⁹, all of which compound cardiometabolic risk.

Each of these pathways converges on brain tissue health—damaging perfusion, promoting inflammation, and degrading microstructure. The cumulative effect should manifest as accelerated biological brain aging: a brain that looks older than its chronological age. Yet no study has tested this prediction directly using multimodal brain age modeling, nor identified the specific vascular and inflammatory mechanisms that drive accelerated aging in lonely individuals.

Multimodal Brain Age Gap: Quantifying How Fast the Brain Ages

Brain age gap (BAG)—the difference between predicted brain age (from neuroimaging) and chronological age—has emerged as a powerful biomarker of biological aging²⁰. A positive BAG indicates a brain that appears older than expected; a negative BAG indicates a brain that appears younger. BAG predicts mortality, cognitive decline, and dementia risk independently of chronological age²¹. Most brain age models use structural MRI features alone. We propose a multimodal brain age model that incorporates features from three independent MRI modalities—ASL perfusion, diffusion-derived free water, and structural T1w/T2w ratio—capturing vascular, inflammatory, and microstructural dimensions of brain aging simultaneously.

Three Biomarkers, Three Stages of the Cascade

ASL-CBF (perfusion). Arterial spin labeling uses magnetically labeled arterial blood water as an endogenous tracer to quantify regional cerebral blood flow²². CBF declines with age and is reduced in AD-vulnerable regions. It indexes the upstream vascular insult from chronic sympathetic hyperactivation and vasoconstriction.

Free water fraction (FW). Free water imaging²³ decomposes the multi-shell diffusion signal to estimate extracellular water unhindered by tissue membranes. Elevated FW in brain tissue reflects an expansion of the extracellular space driven by neuroinflammatory processes including edema, gliosis, and neuronal loss—processes that are downstream consequences of chronic inflammatory signaling and reduced perfusion.

T1w/T2w ratio. The ratio of T1-weighted to T2-weighted signal provides a contrast sensitive to cortical myelin content and tissue microstructure²⁴. Lower T1w/T2w values indicate demyelination and tissue degradation—the downstream microstructural consequence of sustained vascular and inflammatory insult.

Together, these three biomarkers span a cascade: reduced perfusion (ASL-CBF) → extracellular expansion and neuroinflammation (FW) → cortical tissue degradation (T1w/T2w). They serve a dual purpose: (1) as features in the multimodal brain age model, and (2) as mechanistic biomarkers explaining why the lonely brain ages faster. Figure 1 illustrates this conceptual model.

Planned Analyses

Outline analysis pipeline, preprocessing steps, statistical tests, and hypothesis-specific predictions. Include any contingent analysis decisions as IF-THEN statements.

Hypotheses

H1 (Brain Age Gap): Higher loneliness and greater social isolation are associated with a larger positive brain age gap (BAG), indicating biologically older-looking brains.

H2 (Perfusion): Higher loneliness is associated with lower ASL-CBF in AD-vulnerable brain regions (hippocampus, entorhinal cortex, PCC/precuneus).

H3 (Neuroinflammation): Higher loneliness is associated with higher free water fraction in the same AD-vulnerable regions, reflecting extracellular expansion driven by neuroinflammatory processes.

H4 (Cortical microstructure): Higher loneliness is associated with lower T1w/T2w ratio in AD-vulnerable cortical regions, reflecting demyelination and tissue degradation.

H5 (Mechanistic mediation): ASL-CBF, FW, and T1w/T2w mediate the relationship between loneliness and BAG—that is, these biomarkers explain why the lonely brain ages faster.

H6 (Independence): The loneliness–brain aging associations persist after controlling for sex, IQ (NIH Toolbox Cognition Crystallized Composite), depression, perceived stress, sleep quality, BMI, blood pressure, and physical activity.

Impact Statement

This study addresses a fundamental question: does loneliness literally age the brain, and if so, through what biological pathways? If confirmed, we would provide: (a) the first evidence that loneliness increases brain age gap—a finding interpretable in intuitive units (“lonely adults’ brains look X years older”); (b) specific mechanistic targets (perfusion, inflammation, myelin integrity) for loneliness intervention trials; and (c) a multimodal brain age model available for community reuse. If null, demonstrating that loneliness does not accelerate brain aging despite its mortality impact would redirect the field toward peripheral cardiovascular mechanisms. If partially confirmed (e.g., BAG effects without regional biomarker associations, or vice versa), this would clarify whether loneliness operates through distributed vs. focal brain changes. Any result is high-impact in a field where mechanistic neuroimaging data is critically lacking.

3. Methods

Participants/Sample

Dataset. The Human Connectome Project-Aging (HCP-A)²⁵, accessed through the NIMH Data Archive (NDA). HCP-A contains multimodal MRI and comprehensive behavioral assessments from ~725 adults aged 36–100+, scanned on Siemens 3T Prisma scanners at four sites. Release 2.0 includes 1,390 participant sessions.

Inclusion criteria. (1) Completed NIH Toolbox Loneliness assessment; (2) available T1w and T2w structural scans (already downloaded and processed by the PI); (3) available multi-shell diffusion MRI (b = 1500, 3000 s/mm²) passing QC; (4) available PCASL scan with usable perfusion data.

Exclusion criteria. (1) Diffusion MRI with >15% volumes flagged for excessive motion (>2 mm framewise displacement); (2) ASL data with poor labeling efficiency or severe motion artifacts; (3) FreeSurfer parcellation failures; (4) known major neurological conditions (e.g., large stroke, tumor).

Expected sample. Based on typical QC rates and the multi-modal requirement, we anticipate N ≥ 500.

Sample size justification. With N = 500, α = 0.05 (FDR-corrected), and 10 covariates in the full model (H6), we have >95% power to detect small-to-medium effects (f² = 0.03, r ≈ 0.17). For the BAG analysis (H1), brain age gap studies typically report effect sizes of r = 0.15–0.30 for psychosocial predictors²⁰, well within our detection range. For the brain age model itself, N = 500 is sufficient for training a ridge regression or Gaussian process model with cross-validation, as demonstrated in comparable HCP-based studies.

Materials and Procedures

Neuroimaging Acquisition (HCP-A Protocol)

All HCP-A participants were scanned on Siemens 3T Prisma scanners with 32-channel head coils²⁶:

T1w MPRAGE: 0.8 mm isotropic, navigator-corrected. T2w SPACE: 0.8 mm isotropic, navigator-corrected. Both already downloaded and processed through FreeSurfer by the PI.

Multi-shell diffusion MRI: b = 1500, 3000 s/mm², ~100 directions/shell, multiband acceleration.

PCASL: Pseudo-continuous ASL with background suppression; whole-brain CBF maps.

Behavioral Measures

Primary predictor: NIH Toolbox Loneliness Fixed Form (5 items; convergent validity r = 0.86 with UCLA Loneliness Scale)²⁷. Social isolation composite: NIH Toolbox Emotional Support, Friendship, Perceived Hostility, Perceived Rejection. Cognition: NIH Toolbox Cognition Composite (H5 mediation); Crystallized Composite as IQ proxy (covariate). Covariates: Sex, IQ (Crystallized Composite), PSQI, NIH Toolbox Perceived Stress, depressive symptoms (ASR), BMI, blood pressure, physical activity, age, education, race/ethnicity, acquisition site.

Image Processing Pipeline

ASL-CBF. Mean CBF maps from HCP-A preprocessed ASL data, implementing the single-compartment kinetic model²² with tissue-type partial volume correction. ROI-level CBF extracted via FreeSurfer parcellations registered to ASL space.

Free water. Multi-shell diffusion preprocessing: MP-PCA denoising²⁸, Gibbs correction²⁹, eddy current/motion correction (FSL eddy), N4 bias correction. FW estimated via Pasternak bi-tensor model²³. ROI-level FW extracted in native diffusion space.

T1w/T2w ratio. Computed following Glasser & Van Essen²⁴: bias-corrected T1w and T2w images co-registered and divided voxel-wise. Sampled at cortical midthickness surface. ROI-level values from FreeSurfer Desikan-Killiany parcellations. Note: T1w and T2w data are already downloaded and processed through FreeSurfer.

Brain Age Model

A multimodal brain age prediction model will be trained to predict chronological age from neuroimaging features. Features: ROI-level ASL-CBF, FW, and T1w/T2w across all Desikan-Killiany regions (bilateral; ~70 cortical + subcortical ROIs × 3 modalities = ~210 features), plus global metrics (total CBF, mean cortical FW, mean cortical T1w/T2w, total gray matter volume, total white matter volume). Model: Ridge regression with nested cross-validation (outer: 10-fold for BAG estimation; inner: 5-fold for hyperparameter tuning), which is robust to multicollinearity among features and avoids overfitting. We will also evaluate Gaussian process regression as an alternative. BAG computation: For each participant, BAG = predicted age – chronological age, computed from the held-out fold to avoid data leakage. A positive BAG indicates a brain that looks older than expected. Bias correction: BAG will be corrected for the known age-dependent bias in brain age prediction (Le et al., 2018) by regressing out chronological age from the BAG estimates.

Planned Analyses

Brain Age Gap Analysis (H1)

Train the multimodal brain age model via nested 10-fold cross-validation. Compute bias-corrected BAG for each participant. Test the association between loneliness (continuous) and BAG using linear regression controlling for sex, IQ (Crystallized Composite), education, and site. Report the effect in interpretable units: years of additional brain aging per standard deviation increase in loneliness. Repeat with social isolation composite.

Regional Biomarker Analyses (H2–H4)

Separate multiple linear regression models testing loneliness → ASL-CBF, loneliness → FW, and loneliness → T1w/T2w in each ROI. All models control for age (with quadratic term), sex, IQ, education, and site. For ASL-CBF models, global CBF is included as an additional covariate; for T1w/T2w, mean cortical T1w/T2w. FDR correction (Benjamini-Hochberg) across 5 ROIs within each biomarker.

Mechanistic Mediation (H5)

Causal mediation analysis (R mediation package) testing whether regional biomarkers mediate the loneliness → BAG relationship. Path: loneliness → ASL-CBF/FW/T1w/T2w (mediator) → BAG (outcome). Bootstrap CIs (10,000 iterations). Sequential mediation (loneliness → CBF → FW → BAG) will test the proposed cascade ordering.

Independence Testing (H6)

Hierarchical regression testing whether loneliness–BAG and loneliness–biomarker associations survive sequential addition of: (Step 1) demographics + sex + IQ; (Step 2) depression + perceived stress; (Step 3) sleep quality; (Step 4) cardiometabolic (BMI, BP); (Step 5) physical activity.

Contingent Analysis Decisions

IF ASL-CBF quality is substantially lower in the oldest participants (age >80), THEN age-stratified analyses (36–64, 65–79, 80+) and age×loneliness interaction.

IF loneliness distribution is highly skewed (>|2|), THEN additionally dichotomize (top quartile vs. remainder).

IF N < 400 after multi-modal QC, THEN reduce covariates to age, sex, IQ, depression.

IF sex moderates associations (interaction p < 0.05), THEN sex-stratified secondary analyses.

IF site effects persist, THEN ComBat harmonization; report both results.

IF brain age model performance is poor (MAE > 8 years or r < 0.70), THEN report unimodal brain age models separately and evaluate which modalities contribute most to prediction.

4. Ethics and Data Management

Ethical approval. HCP-A approved by Washington University IRB. All participants gave written informed consent. This secondary analysis of de-identified NDA data requires no additional IRB approval. PI holds active NDA Data Use Certification.

Confidentiality. Data stored on encrypted, access-controlled MRRC servers. No re-identification attempts. Derived maps shared publicly contain no PII.

Data sharing and archiving. All code (preprocessing, brain age model, statistics) released on GitHub (MIT license). Derived ASL-CBF, FW, T1w/T2w maps and brain age predictions shared via NDA. Analysis scripts archived on OSF. Preprint on ResearchHub and bioRxiv.

5. Pilot Data

Two lines of published work from the PI provide direct scientific premise and method feasibility:

Pilot Study 1: Neural correlates of loneliness (McKenna et al., Psychological Medicine, 2025). In an fMRI study of 100 participants, we demonstrated that loneliness is significantly associated with altered hippocampal responses to dynamic social stimuli⁸. Higher loneliness predicted greater hippocampal activation to withdrawing faces, surviving control for depression, social anhedonia, and social activity. Hippocampal responses predicted loneliness one year later. Relevance: Establishes measurable neural correlates of loneliness in the hippocampus—our primary target ROI.

Pilot Study 2: Free water in psychotic spectrum disorders (McKenna et al., Schizophrenia Bulletin, 2023). We developed a multi-compartment diffusion model incorporating free water estimation, demonstrating sensitivity to microvascular and microstructural alterations in psychiatric populations⁷. Relevance: Validates our free water pipeline and demonstrates biomarker sensitivity in populations with psychosocial risk factors.

Ongoing work: Einstein Aging Study (EAS) and HCP-A structural processing. We are processing free water maps from the EAS cohort (N ≈ 240 older adults), confirming pipeline reliability at scale. Additionally, T1w and T2w structural data from HCP-A have already been downloaded and processed through FreeSurfer, with parcellations ready for ROI extraction. This substantially de-risks the proposed project timeline.
