Abstract Regional cerebral glucose metabolism, an index of neuronal activity, was compared in 20 patients with Alzheimer's disease and 8 age‐matched normal volunteers by positron emission tomography following { 18 F}2‐fluoro‐2‐deoxy‐D‐glucose administration. Overall cortical glucose utilization in the Alzheimer's group was 10 to 49% below that of control individuals. The posterior parietal cortex and contiguous portions of posterior temporal and anterior occipital lobes were most severely affected; frontal cortex was relatively spared. This pattern of cortical involvement is consistent with the major clinical features of Alzheimer's disease. Comparison of patients with early and more advanced dementia suggested that a substantial decline in glucose metabolism occurs before cognitive impairment becomes evident; once the patient is symptomatic, however, small additional metabolic decrements are associated with a marked deterioration in intellectual function.

Brain tumor metabolism was studied with hydrogen-1 magnetic resonance spectroscopy and positron emission tomography with fluorine-18 fluorodeoxyglucose in 50 patients. N-acetylaspartate (NAA) was generally decreased in tumors and radiation necrosis but was somewhat preserved at neoplasm margins. Choline was increased in most solid tumors. Solid high-grade gliomas had higher normalized choline values than did solid low-grade gliomas (P < .02), but the normalized choline value was not a discriminator of tumor grade, since necrotic high-grade lesions had reduced choline values. Serial studies in one case showed an increase in choline as the glioma underwent malignant degeneration. Choline values were lower in chronic radiation necrosis than in solid anaplastic tumors (P < .001). In two cases studied before and after treatment, clinical improvement and a reduction in choline followed therapy. Lactate is more likely to be found in high-grade gliomas, but its presence is not a reliable indicator of malignancy.

Mathematical methods are of central importance in the new technologies of radiographic and radioisotopic image reconstruction. The most important procedures are classified as Back-projection, iterative, and analytical (Two-dimensional Fourier, Filtered Back-projection). Back-projection played an important historical role but is no longer used because of sizable artifacts. Analytical methods excel in speed and accuracy when a large number of projections are available and are extensively used in x-ray imaging. Iterative reconstruction is more attractive when the number of views is limited, if noise is significant, and if additional factors, e.g., gamma-ray attenuation, are present. For these reasons, iterative methods are widely used in radioisotope imaging.
